Community Living Connections provides a wide spectrum of support services to
adults with developmental disabilities. Our supports range from drop by support
to around the clock support within the home.
Here's how we will proceed:

Upon initial contact from you, we will answer all your questions and tell you
about Community Living Connections.

We will thoroughly listen and then discuss how we can
work together to meet your individual needs.
Here are some of the things that may discussed:

The relationships you value and how we may support you in the development and
maintenance of those.

The people you want involved in decisions regarding your life.

The supports you receive from Community Living Connections.

How you desire to spend your time and the general
structure of your day.

How much, if any, support you currently need regarding medical concerns,
personal care needs and activities of daily living.

Where you would like to live and what are important necessities in your life
for happiness. What are the non-negotiables in your life (i.e. Non-smoking
environment).

How involved you want to be in your community.
-
What your transportation needs are and how that may
effect where you live.
-
How you choose to pursue your spiritual beliefs and any
support needs you may have in that area.

Any assistance you may require in remaining safe in
your home and/or community.

I mainly work with one consumer but through the activities I have met a number
of consumers and they are all fun.

We commit to you that within 30 days we will develop an
individualized plan of support for you. Your support plan will be used as a key
tool in the ongoing guidance and training of the employees of Community Living
Connections, whose role will be to provide support to you as your plan
dictates. The staff of Community Living Connections will be involved in your
self directed team meetings. These meetings will help evaluate how expectations
are being met.
From the start, you will find your experience with
Community Living Connections to be different and unique from other residential
support providers .
Here is a brief description of some of the

individuals participants and employees. It is vital to us that we are
inclusive. We do this by encouraging families and entire support teams to
participate in evaluating our ongoing support.

all that we do in providing our support services. Employees of Community Living
Connections are well informed and committed to the idea of individual choices.
Attention to the desires is crucial to the success of our relationships.
We encourage your personal choices in relationships including those with the
staff that will provide support to you, how you spend your personal time and in
choosing the supports you recieve in the course of your daily life.

Creating relationships with others in your community,
both on a personal and professional basis, are an important part of life.
Community Living Connections supports and fosters these relationships.

Developing close personal relationships and the
locating of community professionals and resources are an important part of your
well-being in the community. We will support the relationships you have
established and will help you locate and connect with new individuals and
resources you desire.

Maintaining relationships with those already important
to you are also an important part of what we can do to assure your personal
desires are supported.

Creating opportunities to participate in or experience
community events that may be new to you.

Creating opportunities to participate in some smaller
well-supported events for those who prefer a more structured and supported
social environment.
